Prepare for the NHS Continuing Healthcare assessment
Describe your relative's needs in your own words. Get a clear statement for each of the 12 assessment areas before the MDT meeting.
In 2025/26 about 1 in 6 standard assessments in England (17.4%) ended with the person found eligible, according to NHS England figures. Clear evidence of the actual needs matters.
How CHCexpert helps you prepare for the CHC assessment
The assessment team records needs, not diagnoses. Families see the needs that the notes miss. This puts them in front of the team in the right shape.
Tell us what happens
Answer simple questions for each area of care, day and night: what you see, how often, how many people are needed and what happens if help is late.
We write the statement
Our system turns your words into a statement for each of the 12 areas, shaped to how the Decision Support Tool describes needs, with the gaps the team is likely to ask about.
Take it to the meeting
Read it, change anything that is not exactly right and share it with the assessor or take it to the MDT meeting. Ask for your view to be recorded in the DST.
The 12 areas of the Decision Support Tool
Each area has its own range of levels, from no needs up to high, severe or priority. Four areas can reach a priority level.
The four characteristics behind the decision
The team also looks at the needs as a whole. Any one of these four, alone or with the others, can show a primary health need.
Nature
What kind of needs they are and what kind of help they call for, for example care that needs skilled hands or trained staff.
Intensity
How much help is needed, how severe the needs are and how continuous they are, day and night.
Complexity
How the needs show up and affect each other, so that more skill is needed to manage them safely.
Unpredictability
How much the needs change without warning and what happens if help is not there in time.
Well-managed needs are still needs. If something is under control only because of the care given, the statement says what that care prevents.
Two packs, one payment each
Write one area free first. When you are ready, the pack writes every area and everything you need around the meeting. It arrives as two PDFs: the papers to use at the meeting, and a guide to prepare.
