NHS Continuing Healthcare is a package of ongoing care arranged and funded solely by the NHS for an adult assessed as having a primary health need. Eligibility depends on the person's actual day-to-day care needs, not their diagnosis, and a multidisciplinary team records those needs in the 12 areas of the Decision Support Tool before the integrated care board decides.
